Flier Distribution Approval Form

Please complete this form and attach two copies of your flier to it. Place it in the president's folder in the PTA box. Once approved by the president, it heads to Mr. Ryan's desk for approval and you will be called once approved.


After School & Evening Building Use Form

Please complete this form if your committee is planning an event after school hours or in the evening and return it to Barbara Barbagallo in the Board of Education office.


Daytime Building Use Form
